Additional Covid Visitor Access Requirements at Pentagon

The Pentagon Force Protection Agency will be enacting new requirements for visitor access to the Pentagon, as directed by Force Health Protection Guidance (Supplement 23), Revision 1, dated Oct. 18, 2021. All visitors entering the Pentagon or any facility protected by the Pentagon Force Protection Agency must present a completed DD Form 3150, "Contractor Personnel and Visitor Certification of Vaccination," prior to entry, beginning Monday, Nov. 1, 2021.

Visitors can download the DD3150 here: https://www.esd.whs.mil/Portals/54/Documents/DD/forms/dd/dd3150.pdf. Please read the instructions on the form carefully. Any visitor who indicates they are Not Yet Fully Vaccinated, Not Vaccinated, or Declines to Respond will, in addition to the DD3150, be required to provide a negative COVID test result (from either a rapid test or a PCR test) dated no more than 72 hours prior to the visit or they will be denied entry.
