New Centrelink Medical Certificate now live

Services Australia have revised the Centrelink Medical Certificate (SU415) to make it easier to complete.

The changes are in response to feedback from the AMA and health practitioners. It now should take less time to complete the certificate.

The new certificate clarifies:

  • the information you need to give to Services Australia

  • terminology such as medical exemptions are based on temporary incapacity regardless of whether a medical condition is temporary or permanent

  • details needed by Services Australia for easier identification of patients with a serious illness or a terminal condition.

The certificate also includes more details to:

  • reduce the need for rework and extra time to complete the certificate

  • help identify patients who may be manifestly eligible for Disability Support Pension.

You can download it from the Services Australia website, complete within HPOS and upload, or request printed pads. It's preferred you use the Centrelink Medical Certificate to ensure the required information is included.

You can complete the certificate on a device, or print and complete by hand. You should always sign the certificate by hand unless uploading via HPOS.

Please note that if Services Australia deem the certificate incomplete, it will require further consultation and completion.
