Our community health dietitians work across our District to improve nutritional care in the community.

Providing support in client’s homes and at community health centres, our dietitians provide nutrition assessments and screening and nutrition support through diets, supplements and nutrition care plans.

Senior Community Health Care Dietitian, Rachel Alexander says, “The majority of our work is conducted in our client’s homes through home visits. We complete nutritional assessments as part of a holistic assessment to ensure consumer’s needs are being identified and treated.”

With a wide range of adult clients with chronic and complex health conditions such as COPD, motor neurone disease, gastrointestinal conditions, and wounds; their main role is addressing issues around malnutrition.

“We also manage clients with enteral feeds, and support clients on the transitional aged care program with aims of nutrition support for restorative care,” added Nilofar Dorani, Community Health Care Dietitian.

The support and guidance provided by our community health dietitians help to prevent admissions to hospital or aged care facilities.

“Helping to increase nutritional knowledge and addressing nutritional issues through home visits helps our clients remain comfortable at home and have the reassurance we are there to help them,” says Lauren McGrath, Dietitian.
