Police are seeking public assistance to help locate missing Waubra man Dimitro Zelenko.
The 73-year-old was last seen leaving a medical facility on Ascot Street South in Ballarat about 4.30pm on Monday 2 November.
Police and family have concerns for his welfare as he has a medical condition that requires regular medication which he does not have with him.
Dimitro may be travelling in a 2002 blue Toyota Landcruiser with registration ECHOS1.
Investigators have released an image of Dimitro in the hope someone recognises him and can provide information regarding his current whereabouts.
He is known to frequent Waubra, Ballarat and St Kilda.
Anyone with information about Dimitro is asked to contact Ballarat Police Station on 5336 6026.
