Police are appealing for public assistance to help locate missing teenager Lachlan.
The 17-year-old was last seen leaving Geelong Hospital about midday.
Lachlan is described as 183cm tall, thin build, medium length dark hair and unshaven.
He was last seen a grey hoody, dark blue pants and black runners.
Police and family have concerns for his welfare due to medical conditions.
Police have released an image of Lachlan in the hope someone recognises him and can provide information on his current whereabouts.
Anyone who sees Lachlan is urged to call Geelong Police Station on 5225 3100.
