Police are appealing for public assistance to help locate missing man Sam.
The 75-year-old was last seen at a care facility in Rymill Court, Altona North about 5.45pm on Sunday 23 June.
Police and family have concerns for his welfare due to his age and medical conditions.
Sam was last seen wearing a white shirt with black sleeves and grey track pants.
Investigators have released an image of Sam in the hope that someone can provide information on his current whereabouts.
